Blood pressure after menopause: why it starts to climb
Blood pressure rises in women around and after the menopause transition, and the prevalence of hypertension in women overtakes that in men from around the sixth decade. Estrogen supports blood vessel flexibility through nitric oxide and restrains the renin-angiotensin system; as it falls, vessels stiffen and sodium handling changes. High blood pressure causes no symptoms until it causes damage, which is why it is the most valuable thing to measure in this decade.
The pattern
Through the reproductive years, women generally have lower blood pressure than men of the same age. That advantage narrows across the menopause transition and then reverses: from around the sixth decade, hypertension is more prevalent in women.
Some of this is ageing, which raises blood pressure in everyone. The evidence suggests a menopause-specific component on top of it, supported by studies separating the two and by the earlier rise seen in women who reach menopause early.
The mechanisms
Estrogen promotes nitric oxide production in the lining of blood vessels, which causes them to relax. Less estrogen means less nitric oxide signalling and stiffer, less responsive arteries.
Estrogen also restrains the renin-angiotensin-aldosterone system, which governs sodium and fluid balance. With less restraint, that system becomes more active and sodium is retained more readily, which raises pressure and makes blood pressure more salt-sensitive than it was.
Weight redistribution to the abdomen contributes independently, and disrupted sleep is an under-appreciated factor: both insomnia and obstructive sleep apnoea, which becomes more common after menopause, raise blood pressure directly.
Why it is missed
High blood pressure produces no symptoms. The occasional headache or nosebleed attributed to it is usually unrelated, and waiting to feel something is the reason it goes undetected for years.
In this age group there is a second problem: palpitations, headaches, flushing, poor sleep and anxiety are all attributed to hormones, and a blood pressure cuff is often not reached for during a menopause consultation.
Women are also less likely to have cardiovascular risk factors identified and treated to target than men with the same numbers, a gap documented repeatedly.
What is worth knowing about measurement
A single reading in a clinic is a poor basis for anything. Home readings over several days, or ambulatory monitoring over 24 hours, give a far better picture, and many diagnoses are confirmed or excluded this way.
Cuff size matters and a cuff that is too small overestimates. This is a common and easily corrected source of error.
If you have had pre-eclampsia or gestational hypertension in any pregnancy, your long-term cardiovascular risk is higher. It should be part of every risk conversation and is very rarely asked about.
When to see someone
Get blood pressure checked around the menopause transition even with no symptoms, and ask for it alongside lipids, HbA1c and waist measurement.
Seek urgent care for a very high reading with chest pain, breathlessness, severe headache, visual disturbance, confusion or weakness on one side.
