Cholesterol changes after menopause: what shifts and why
Cholesterol changes measurably around the final menstrual period, independent of ageing. LDL and total cholesterol rise, often sharply in the year before and after, and triglycerides tend to increase while HDL becomes less protective in function even when its level holds. This is part of why cardiovascular risk in women rises after menopause towards the level seen in men, and why a lipid check around this time is worth asking for.
A change tied to menopause, not just to age
Longitudinal studies following the same women through the transition have been able to separate the effect of the menopause itself from the effect of getting older, and they find a distinct shift in lipids clustering around the final menstrual period rather than tracking age smoothly.
The typical pattern is a rise in total and LDL cholesterol concentrated in roughly the year either side of the final period, with apolipoprotein B rising alongside.
This is one of the clearest pieces of evidence that menopause changes cardiovascular risk rather than merely coinciding with the age at which it rises.
Why estrogen matters to lipids
Estrogen increases the expression of LDL receptors in the liver, which are what clear LDL particles from the blood. Fewer receptors means slower clearance and higher circulating LDL.
Estrogen also influences triglyceride handling and the composition and function of HDL particles.
The HDL point is worth isolating because it is counterintuitive: HDL concentration may hold steady or even rise slightly after menopause, while its functional capacity to remove cholesterol from tissue declines. A reassuring HDL number can therefore overstate how protected someone is.
What else is happening at the same time
Lipids do not change in isolation. The same period brings the shift of fat towards the abdominal and visceral compartments, declining insulin sensitivity, and rising blood pressure.
These cluster, and they compound: visceral fat drives both the lipid pattern and the insulin resistance. It is why waist circumference belongs in the same conversation as a cholesterol result.
What to do with the information
The useful output is a risk assessment rather than a single number. Most health systems use a calculator combining age, blood pressure, lipids, smoking, diabetes and family history to estimate cardiovascular risk over the next ten years, and that estimate is what treatment decisions turn on.
Risk calculators have historically underestimated risk in women, and factors specific to women — early menopause, pre-eclampsia, gestational diabetes, polycystic ovary syndrome — are recognised as raising risk while not appearing in most calculators. They are worth mentioning explicitly.
Hormone therapy is not prescribed to lower cholesterol and is not a cardiovascular prevention treatment, though oral preparations do affect lipid levels. Timing relative to menopause affects its cardiovascular profile, which is a discussion with a clinician.
When to see someone
A lipid profile around the time of the menopause transition is worth asking for even if you have had one before, because this is precisely when the numbers move.
Ask for it alongside blood pressure, HbA1c and waist circumference, and mention early menopause or any pregnancy complications, since both change the risk estimate and neither is usually asked about.
